What BMI Is (And the Surprising Reason It Was Invented)
BMI was developed in the 1830s by Belgian mathematician Adolphe Quetelet — not a doctor. He created it to describe the statistical distribution of body weight across populations, not to assess individual health. The formula was:
BMI = Weight (kg) ÷ Height² (m²)
For someone who is 170 cm tall and weighs 70 kg: BMI = 70 ÷ (1.70)² = 70 ÷ 2.89 = 24.2
That number gets compared to a classification table.

How to Calculate BMI in Both Unit Systems
Metric (kg and cm) — Used in India and Most of the World
BMI = Weight in kg ÷ (Height in meters)²
Example: 65 kg, 165 cm (1.65 m) BMI = 65 ÷ (1.65)² = 65 ÷ 2.7225 = 23.9 (Normal weight)
Imperial (lbs and inches) — Used in the US
BMI = (Weight in lbs ÷ Height in inches²) × 703
Example: 143 lbs, 65 inches (5'5") BMI = (143 ÷ 65²) × 703 = (143 ÷ 4,225) × 703 = 0.03385 × 703 = 23.8 (Normal weight)
Both give the same result (within rounding) — just different units.
The Asian BMI Thresholds: Why Your Chart May Be Wrong for You
Standard BMI thresholds were developed primarily from studies on Caucasian (Western European) populations. Substantial research since the 1990s has shown that Asian populations — including Indians — develop metabolic complications (type 2 diabetes, hypertension, cardiovascular disease) at lower BMI values.
WHO Asia-Pacific and Indian guidelines:

What this means practically: An Indian person with a BMI of 23.5 would be classified as "normal weight" by Western charts but as "overweight/at-risk" by Asian guidelines.

Who BMI Gets Wrong
Athletes and Highly Muscular People
Muscle is denser than fat. An elite athlete with 8% body fat and significant muscle mass will have a higher BMI than a sedentary person of similar height — sometimes pushing into the "overweight" range despite being in excellent health.
Example: Virat Kohli is reported to be approximately 175 cm tall and 69 kg. BMI ≈ 22.5 — normal range. But many heavyweight strength athletes or rugby players at 190 cm and 110 kg would have BMI ≈ 30.5 (obese category) despite extremely low body fat.
BMI cannot distinguish between fat mass and lean mass. It measures neither. It measures total weight relative to height.
Elderly People
With age, muscle mass decreases (sarcopenia) while fat mass tends to increase — sometimes without significant weight change. An elderly person can have a "normal" BMI while having dangerous levels of visceral fat and low muscle mass.
For older adults (65+), some research suggests that a BMI in the 25–27 range may actually be associated with better health outcomes than a BMI below 23 — counter to the standard chart.
Short People
The BMI formula doesn't scale correctly at extreme heights. Very short individuals tend to have their BMI overestimated (appearing heavier than the health risk warrants), while very tall individuals tend to have BMI underestimated.
Pregnant Women
BMI is not applicable during pregnancy. Gestational weight gain guidelines use pre-pregnancy BMI as a baseline, not current weight.
Better Numbers to Know Alongside BMI
Waist Circumference
Abdominal fat (visceral fat around organs) is more metabolically dangerous than fat stored elsewhere. Waist circumference captures this.

Formula: Waist circumference ÷ Height (both in same units)
Target: Below 0.5 for most adults. "Keep your waist to less than half your height" is the simplified rule.
Example: Waist 85 cm, height 170 cm → WHtR = 85/170 = 0.50 (at the boundary)
Research suggests WHtR outperforms BMI at predicting cardiovascular risk across different ethnicities — it's naturally self-adjusting for height in a way BMI isn't.
Body Fat Percentage
The most direct measure of adiposity. Methods to measure it range from simple to complex:
- Skinfold calipers: Inexpensive, requires technique
- Bioelectrical Impedance Analysis (BIA): Consumer smart scales with BIA (Xiaomi, Tanita, Fitbit) — convenient but affected by hydration
- DEXA scan: Gold standard; available in some hospitals and diagnostic centers; most accurate
- Hydrostatic weighing: Very accurate; limited availability

BMI for Children: A Different Scale
For children and teenagers (ages 2–19), BMI is calculated the same way but interpreted differently — using age- and sex-specific percentile charts because body composition changes significantly during growth.

BMI is a reasonable starting point for a conversation with a doctor — not a verdict. Here's how to use it productively:
If your BMI is in the normal range:
- Confirm with waist circumference (should be below the risk threshold)
- Get blood markers checked periodically: fasting glucose, HbA1c, lipid panel, blood pressure
If your BMI indicates overweight or obese (by appropriate scale for your ethnicity):
- Don't panic — it's a population-level screening flag, not a diagnosis
- Add waist circumference and, if possible, body fat percentage for a more complete picture
- A 5–10% body weight reduction has been shown to meaningfully improve metabolic markers regardless of starting BMI
If your BMI indicates underweight:
- Investigate root causes: nutritional gaps, thyroid function, other underlying conditions
- Muscle mass loss (sarcopenia) can coexist with normal or even high BMI in older adults
