Reading Blood Pressure: What the Two Numbers Mean
Blood pressure is expressed as two numbers separated by a slash: Systolic / Diastolic, measured in mmHg (millimetres of mercury).
Systolic pressure (top number): The maximum pressure in your arteries when your heart muscle contracts and pushes blood out. This is when the heart "beats."
Diastolic pressure (bottom number): The minimum pressure in your arteries between heartbeats — when the heart muscle relaxes and refills with blood.
Example: 120/80 mmHg
- Systolic: 120 mmHg (peak pressure during heartbeat)
- Diastolic: 80 mmHg (resting pressure between beats)
Both numbers matter independently. Isolated systolic hypertension (high systolic, normal diastolic) is the most common form in people over 60 and carries significant cardiovascular risk. Isolated diastolic hypertension (elevated diastolic only) is less common and less studied.
Blood Pressure Categories: The Updated Classification
The American College of Cardiology (ACC) and American Heart Association (AHA) updated blood pressure categories in 2017. These are now widely adopted, including by the Cardiology Society of India.

The shift from 140/90 to 130/80:
Until 2017, hypertension was universally defined as 140/90 or above. The 2017 reclassification moved Stage 1 Hypertension down to 130/80. This was controversial because it significantly increased the prevalence of hypertension — but was justified by evidence showing cardiovascular risk increases even at 130–139/80–89.
For Indian context: Indians have higher cardiovascular risk at comparable blood pressures compared to Western populations — lower BMI thresholds for metabolic risk are well established. The 130/80 threshold is therefore particularly appropriate for Indian patients.
How to Measure Blood Pressure Accurately
A single blood pressure reading has significant measurement error. Getting accurate readings requires understanding the factors that influence the number:
Preparation Matters Enormously
Do before measuring:
- Sit quietly for 5 minutes before the reading
- Empty your bladder (a full bladder raises BP by 10–15 mmHg)
- Sit upright with back supported and feet flat on the floor
- Rest your arm at heart level on a flat surface
- Ensure the cuff is on bare skin, not over clothing
Do NOT do within 30 minutes of measuring:
- Exercise
- Drink caffeine (coffee, tea, energy drinks)
- Smoke or use tobacco
- Eat a large meal
Proper technique:
- Cuff wrapped snugly — two fingers should barely fit under the cuff
- Cuff placed 2–3 cm above the elbow crease
- Do not speak during measurement
- Take two readings 1–2 minutes apart; record the average
Home BP monitoring: More accurate than clinic measurements for most people because it eliminates white coat hypertension. ABPM (Ambulatory Blood Pressure Monitoring — a device worn for 24 hours) is the most accurate method and is used to confirm or rule out hypertension when readings are borderline.
White Coat Hypertension: More Common Than Most Think
White coat hypertension — where blood pressure is high in a clinical setting but normal at home — affects 15–30% of people with elevated clinic readings. For these individuals, clinic-measured BP overestimates the true cardiovascular risk.
How to identify it:
- Clinic reading: 145/92
- Home monitoring (morning and evening, 3 days): average 125/78
This person has white coat hypertension, not true hypertension — and would be inappropriately medicated based on clinic readings alone.
Masked hypertension is the opposite — normal clinic readings but elevated home measurements. This affects 10–15% of people and is arguably more dangerous because it's missed. Caused by stress at work, heavy alcohol use, or work-related physical exertion.
Both are diagnosed through ABPM or systematic home monitoring — not clinic readings alone.
Mean Arterial Pressure (MAP): The Calculated Number
MAP is the average pressure in the arteries throughout the cardiac cycle — a single number that better represents the true perfusion pressure to organs.
MAP formula: MAP = Diastolic + (1/3 × Pulse Pressure) Where Pulse Pressure = Systolic − Diastolic
Or simplified: MAP = (Systolic + 2 × Diastolic) ÷ 3
Example — 120/80 mmHg: MAP = (120 + 2 × 80) ÷ 3 = (120 + 160) ÷ 3 = 280 ÷ 3 = 93 mmHg
Normal MAP range: 70–100 mmHg Below 70 mmHg: Inadequate perfusion to vital organs (shock risk) Above 110 mmHg: Associated with significantly increased cardiovascular risk

Pulse Pressure: The Overlooked Indicator
Pulse pressure = Systolic − Diastolic
Normal: 40–60 mmHg
High pulse pressure (> 60 mmHg): Often indicates arterial stiffness — the arteries have lost elasticity and can't dampen the pulse wave effectively. Associated with aging, atherosclerosis, and aortic regurgitation. Cardiovascular risk predictor, especially in older adults.
Example: 160/70 → Pulse pressure = 90 mmHg (very high — isolated systolic hypertension with arterial stiffness)
Low pulse pressure (< 25 mmHg): Indicates reduced stroke volume — heart isn't pumping enough per beat. Can signal heart failure, severe dehydration, or aortic stenosis.
Risk Factors for Hypertension: The Indian Context
India has an estimated 200 million people with hypertension — and fewer than half are aware of it. Less than a third of those diagnosed have it controlled. The scale of undetected and uncontrolled hypertension in India is one of the leading public health challenges.
Risk factors particularly relevant in India:
Diet: High sodium intake (Indian cooking uses significant salt in dal, pickles, papads, namkeens, restaurant food) combined with low potassium (insufficient fruits and vegetables) creates a sodium-potassium imbalance that promotes hypertension.
Physical inactivity: Urbanisation has dramatically reduced occupational physical activity. Indian urban adults have among the lowest physical activity rates globally.
Stress: Urban work pressure, financial stress, and traffic exposure are all associated with elevated BP.
Alcohol: Heavy alcohol consumption (particularly spirits) is strongly associated with hypertension. Men who drink heavily have a 2× higher hypertension risk.
Abdominal obesity: Even at normal BMI, Indians tend to carry more visceral fat, which is more metabolically active and hypertension-promoting.
Family history: Hypertension has strong genetic components — first-degree relatives of hypertensive patients have 2× the risk.
Age: BP rises with age in most populations. Over 60, 60–70% of Indians have hypertension.

When to Seek Medical Evaluation
Immediately (Hypertensive Crisis — BP > 180/120): Seek emergency medical attention if accompanied by:
- Severe headache, chest pain, shortness of breath
- Vision changes, dizziness, confusion
- Back pain (may indicate aortic dissection)
Urgently (within days):
- Consistent home readings above 140/90 on multiple days
- BP above 130/80 with diabetes or kidney disease
- Any BP reading accompanied by symptoms even if not critically high
Routine evaluation:
- First hypertension diagnosis: full metabolic panel, kidney function, urine albumin, lipid profile, ECG
